Changing the Order of Consent Forms

To change the order in which consent forms are displayed within an intake package, follow the steps below:

1. Open the intake questionnaire in the form editor by going to "My Forms" and clicking on the desired intake questionnaire.
2. Click on "Consent Forms".
3. Uncheck all the consent forms that are checked.
4. Now check the consent forms in the order in which you want them to appear.
5. Click on "Save".
