Placeholders in Consent Forms

Z-Forms supports the use of placeholders in consent forms templates. Placeholders are like variables used in form templates,like [PractitionerName], that are replaced by real data when a client form is created.

The following placeholders are currently supported:

  • [ClientName] - will be replaced by the client's full name.
  • [ClientFirstName] - will be replaced by the client's first name.
  • [PractitionerName] - will be replaced by the full name of the practitioner associated with the client form.

Example

The following text in your consent form template:

"I understand that [PractitionerName] from Heathy Clinic Ltda. will have access to my health records."

Will turn into:

"I understand that Dr. Joseph Smith from Heathy Clinic Ltda. will have access to my health records."
