Provider Signature in Consent Forms

Some consent forms need to be signed by the client and the provider. In these cases, it’s possible to indicate that a consent form requires the provider signature.

To do this, check the "Enable Provider Signature" option. You’re also able to add your own text above the signature, by clicking on the "Customize" link.

When the provider signature is enabled, you’ll be able to sign the consent form after it has been submitted. When you navigate to the submitted form page, the consent forms that require your signature will display a sign button beside it.

In the consent form page, you’ll see the box that will capture your electronic signature:

When printing the consent form, the provider signature will go under the client's signature:
